** GROTON-DUNSTABLE REGIONAL MIDDLE SCHOOL PARENT SURVEY**
Dear Parents/Guardians,
As part of our ongoing process in promoting a quality educational program for our students, the GDRMS School Council is conducting a parent survey to identify areas of strength and/or opportunities for improvement in the categories of school climate, student discipline, faculty and administration, home-school communication, parental involvement, curriculum, instruction and assessment, student-led conferences, reporting of student progress, food services, building security and the Title 1 Program. Your opinions and suggestions in how we can better serve the educational needs of our students and families are very important to us. Your responses will continue to help us prepare our goals and objectives for our 2008-2011 School Improvement Plan.
We encourage you to consult with your child before filling out this survey. Please complete the survey as carefully and candidly as possible. Parents are asked to indicate the extent to which they “Agree” or “Disagree” with each of the survey statements. Please click to select the answer which best reflects your opinions for each item. Some questions may not apply to you and your child, so we have given you the opportunity to circle NA (non-applicable).
Part 2 consists of two “Open-Response Questions”. This will give you the opportunity to respond to other questions/concerns. We would also enjoy hearing about things that you have been pleased with!
Your signature is optional. However, for effective data collection, please indicate your child’s grade level on the survey sheet. Please fill out a separate survey for each child who attends the middle school. SurveyMonkey.com only gives you the option to fill out one survey. If you have two or more students in the middle school, please forward your email address to Cheryl Douglas and she will email you an additional survey to fill out.
Once the surveys are in, they will be tallied and shared with staff and faculty. The final results will be posted on our website.
